The state now requires a NON-IVP fingerprint card to obtain or renew a massage therapy license. The information is on the state board website above and the form can be accessed at: https://psp.azdps.gov/fingerprintClearance/formTriage (link is external) Select Apply for a new card” Select Non-IVP” Complete the printable pdf form, and send, or hand carry, the completed application, FD-258 fingerprint card and fee to DPS Licenses now expire 24 months from the date of renewal, not on your birthday. STATE BOARD OF MASSAGE THERAPY MEETINGS The State Board of Massage Therapy meets the 4th Monday of the month at 9 am. The meetings are held on-line.
